Premature shutdown of the main and vernier engines occurred as a result of an electrical malfunction in the vehicle's power bus, specifically a short circuit to ground. A Mishap Board investigation concluded the most probable cause of the failure was flight vibration that induced mechanical damage to wiring insulation resulting in a short to ground.
