Lessons Learned – Capture of Observations from Completed Missions for Post-Flight Follow-up and Lessons Learned

Abstract: Description of Driving Event:
Mission teams tend to be re-assigned to other priorities before a complete post-flight or post-mission follow-up are completed, causing delays and loss of important observations and assessments from each mission. In addition, there is a significant amount of work to completing post-mission reviews that need to be done by the outgoing LPM team.
Root cause: Pressure to re-assign personnel to other priorities immediately at the end of a mission; results in post flight follow-up and lessons learned documentation not being completed in a timely fashion.
