Lessons Learned - Removing the gaseous nitrogen Pressure Control Valve 214(PCV-214) in the A-2 Test Stand Pressure Reducing Area (PRA)

Abstract: Description of Driving Event:
On August 1,1996, contractor personnel were removing the gaseous nitrogen Pressure Control Valve 214(PCV-214) in the A-2 Test Stand Pressure Reducing Area (PRA). As a result of a sudden pressure release of 3000 psi nitrogen, one technician suffered a fractured right forearm, and another suffered a small puncture wound to his right neck area and abrasions to his arm. Both the aforementioned employees and another are being monitored over time to determine if there are any auditory threshold shifts.
